Download, complete and email your completed 'Fitness to Dive Questionnaire' (RSTC form) to applications@dansa.co.za.

Please attach a scanned copy, screen capture or image of your your highest Diving Certification.

New Members please download and complete the 'New Member Application'.

For Members renewing, please download and complete the 'Membership Renewal' form unless:

• Your personal information has changed or is different from your information registered with the DAN Shop.
• Your Next of Kin details have changed.
• You need to add or remove family members from your membership.
• Then download and complete the 'New Member Application'
• Please download the Debit Order Mandate form if you are paying by monthly or annual debit order.

Your application will be processed once all required forms are received by Divers Alert Network Southern Africa.

Your application will take up to 48 hrs to process.

Your application will be processed during normal business hours 08:00 to 16:00 Monday to Friday.
